Low-Dose Radiotherapy for Arthritis: Safety, Process, and Questions
Learn what happens during low-dose radiotherapy for osteoarthritis, who may not be a candidate, and what is known and uncertain about risk.

Short answer: Low-dose radiotherapy for osteoarthritis is usually delivered in six brief outpatient sessions after a consultation and planning visit. Most reported short-term effects are mild, but the evidence for benefit is mixed and long-term radiation risk cannot be described as zero.
Step 1: Confirm the diagnosis
Joint pain has many possible causes, including osteoarthritis, inflammatory arthritis, tendon disease, nerve pain, injury, infection, or referred pain from the spine.
Before radiation is considered, clinicians should review:
- symptom location and pattern;
- physical examination and imaging;
- prior treatments and response;
- medication and health history;
- whether surgery is appropriate;
- age and pregnancy potential; and
- prior radiation to the area.
Radiation should not be used simply because a joint hurts.
Step 2: Consultation and shared decision-making
A radiation oncologist should explain the proposed benefit, conflicting evidence, alternatives, and site-specific risks. This is also the time to define a realistic goal, such as walking farther, sleeping better, or reducing a particular pain score.
Established osteoarthritis care may include exercise or physical therapy, weight management when appropriate, topical or oral medication, braces, injections, and surgery. Low-dose radiotherapy is usually considered after conservative options have not provided adequate relief or are unsuitable.
Step 3: Planning
The treatment team positions the joint and identifies the area to treat. Depending on the joint and equipment, planning may use a simulation image or clinical setup. Shields, beam angles, and field size are chosen to limit unnecessary exposure.
Reproducible positioning matters even though the radiation dose is low.
Step 4: Six outpatient treatments
A commonly described regimen is 0.5 Gy per session for six sessions, totaling 3 Gy over about two to three weeks. Each appointment may take 15 to 20 minutes, while radiation delivery itself is brief.
The treatment:
- is noninvasive;
- does not require anesthesia;
- is not felt as it is delivered; and
- does not make the patient radioactive.
Schedules vary, and treatment should follow an individualized prescription.
Step 5: Follow-up
Relief may develop gradually over weeks to months. Follow-up should use the same pain and function measures recorded before treatment.
A second course is sometimes described in the literature for nonresponders, but it should not be automatic. Persistent symptoms may require diagnostic reassessment, and cumulative exposure should be reviewed.
What short-term effects can occur?
Many studies report few acute effects. Depending on the site, possible effects can include:
- temporary skin redness or dryness;
- short-lived pain flare;
- fatigue;
- localized hair or nail changes; or
- no noticeable reaction.
A 2025 meta-analysis reported more overall adverse events and nail reactions with LDRT, although serious events were not a prominent finding. Patients should receive site-specific instructions rather than being told there are “no side effects.”
What about cancer risk?
Ionizing radiation can cause cancer. At the small doses used for osteoarthritis, the absolute risk is expected to be low, particularly in older adults, but it cannot be measured precisely or called zero.
Risk depends on:
- age at exposure;
- treated body site and organs nearby;
- field size and dose;
- prior radiation; and
- individual susceptibility.
This uncertainty is one reason many experts are more cautious in younger patients and why treatment should be justified against expected benefit.
Who may not be a candidate?
Reasons to avoid or reconsider treatment may include:
- pregnancy;
- active infection in the treatment area;
- uncertain source of pain;
- very young age relative to a benign condition;
- prior radiation that limits safe additional dose;
- a condition needing urgent surgical or medical care; or
- expectations that radiation will restore cartilage or guarantee pain relief.
Each case requires clinical review; this is not a complete contraindication list.

- What confirms osteoarthritis is causing my symptoms?
- Which treatments have I not yet tried?
- What is the best supportive and negative evidence for this joint?
- How will you limit dose to nearby tissue?
- What benefit should I expect, and by when?
- What happens if I do not improve?
- Has my insurance confirmed coverage?
Low-dose radiotherapy can be a reasonable discussion for selected people, but safe care starts with accurate diagnosis, transparent evidence, and a clear follow-up plan.
Questions patients often ask
Is low-dose radiotherapy painful?
Radiation delivery itself is not usually felt. The joint must remain in the planned position for several minutes, which can be uncomfortable for some people.
Will I be radioactive after treatment?
No. External-beam low-dose radiotherapy does not make a person radioactive, and there is no radiation exposure to family members afterward.
How quickly can arthritis pain improve?
Improvement may be gradual over weeks or months, and some people do not respond. The follow-up plan should define when and how benefit will be assessed.
Does insurance cover arthritis radiotherapy?
Coverage varies by plan, diagnosis, policy, and documentation. Approval should be verified before treatment; professional criteria do not guarantee coverage.
